Required Registration, Protection of Privacy
Registration is required for each event and we kindly ask that you aim to complete this form within 48 hours of ticket purchase.
Participants must register using their real names in this registration form so attendance can be privately verified with your choice of pseudonym or non-identifying display name during live Zoom meetings.
For participants using a pseudonym or non-identifying display name, please aim to arrive at least 10 minutes before the meeting begins for check-in so the host can privately match your display name with your registered name. Please use only the display name provided on your registration form or your attendance cannot be verified for admission.
Zoom recordings
Meetings are recorded by Ask Your Therapist for clinical archives and will not be available for replay.
Zoom meeting clinical archives are used for service-quality review. Access to and review of these archives is limited to the host of Ask Your Therapist, Lee Park, owner of Canada Online Therapy.
Participants may take personal written notes but may not audio record, video record, reproduce, or distribute meeting content.
Ask Your Therapist meeting content remains the intellectual property of Lee Park.
Optional Identity Protection
Participants are responsible for protecting their own privacy by not displaying or disclosing identifying information they do not want shared in the meeting.
Participants are asked to keep the identities, personal disclosures, stories, and identifying information of other participants, as well as meeting content, confidential.
Sensitivity Towards Meeting Content and Process
Meetings may include personal questions, sensitive discussions, participant discussion, and sample or concentrated therapy exercises.
Participation is voluntary. Participants may decline to participate in group discussions and may leave at any time.
Although encouraged to notify the host privately in advance, please do not announce to the group you are leaving early, to protect the flow of the group meeting.
Persons are allowed to come as they are, be as they are and express themselves. Profanity is accepted; abusiveness and rudeness are not.
This group is to encourage growth, health, and the safety of its group members. Participants may be removed without notice for behaviour that interferes with integrity, security, predictability of meetings.
Please note, ordinary online-meeting interruptions, including background noise, may occur; for example, the host’s fur babies may rarely be activated, bark, or appear in the frame to naturally say hello.
Respecting the Process
Entry is controlled to maintain the structure of the meeting. Participants arriving more than 10 minutes after the meeting begins will not be admitted.
If a participant cannot arrive on time and the meeting is missed, the ticket may be transferred to a future event by contacting the host in advance. Early departure does not qualify for a ticket transfer.
Ticket credit
Each Live Zoom event ticket purchased separately from membership includes a $20 credit toward one individual, couples, family, child/adult, or group therapy session with Canada Online Therapy.
This credit may be applied toward sliding-scale or regularly priced sessions, and proof of ticket purchase is required.
One ticket credit will be accepted per therapy session.
The credit does not expire.
Consent
By submitting the below registration form, the participant confirms that the information provided is accurate, that they are the person who will attend the meeting, and that they have read and agree to these meeting conditions.
Tickets are non-refundable except in the event of host illness or host cancellation.
